online web-based surveillance to gather traveler information

  Dear Customers, The Government of the United Republic of Tanzania through the ministry of health has introduced  online web-based surveillance to gather traveler information pertaining to the purpose of improving public health. This is in accordance with Public Health Act 2009, thus false information is an offense. Each traveler is required to complete an individual form. Note: Upon submission of the form, the traveler will receive Unique Health Code (UHC) which will be presented during primary screening. We are proud of his achievements, roads, railways, upgrading our airports, and resuscitating our national airline - Air Tanzania Company Limited. We are thrilled that Tanzania is connected directly with India and China through our own, and inevitably we will be connected even further to so many other parts of the world.   He will be missed.
